Depression that hasn’t lifted
This is the reason most patients come to us for TMS. If one or more antidepressants haven’t worked, or haven’t worked well enough, this is likely the conversation to have.
Depression careIf you’ve tried antidepressants and depression is still there, you’re not out of options. TMS is a drug-free, FDA-cleared treatment for depression, OCD, and anxious depression, delivered in-office in North Scottsdale under the supervision of Dr. Roland Segal, MD.
Now accepting new TMS patients · Most start within 2 weeksWhat Is TMS Therapy
Depression that hasn’t responded to medication is exhausting in a way that’s hard to explain to people who haven’t been there. You’ve tried the pills. Maybe two, maybe five. Some helped a little. Some didn’t help at all. Most came with side effects that made you wonder if the trade was worth it.
TMS is different. It’s a drug-free treatment that uses gentle magnetic pulses to reach the exact part of your brain that regulates mood, which is often quieter than it should be in people with depression. There’s nothing to swallow, nothing to metabolize, and none of the weight gain, sexual side effects, or emotional flatness that make so many antidepressants hard to live with.
TMS has been FDA-cleared for major depression since 2008, and later cleared for OCD and depression with anxious features. It’s most often prescribed for adults who’ve tried at least one antidepressant without getting the relief they hoped for. If that sounds like where you are, TMS might be worth talking about.
At our Scottsdale clinic on Raintree Drive, just off Loop 101, sessions happen in a private, quiet room. You sit in a comfortable chair, closer to a dentist chair than a hospital bed, and stay awake the whole time. Read, listen to music, or just rest. When you’re done, you drive yourself home. Call (623) 288-2628 to see if TMS is right for you.
Your TMS physician
Every TMS candidate at our Scottsdale clinic is evaluated by Dr. Roland Segal, our Medical Director and a Managing Partner of SMHWI. He’s double board-certified in General and Forensic Psychiatry, a Clinical Assistant Professor of Psychiatry at the University of Arizona College of Medicine, and past president of the Arizona Psychiatric Society. You’ll get an honest read on whether TMS is likely to help before you commit to anything, and the same team supervising your sessions is the team managing your medication and follow-up.
How TMS Works
TMS isn’t magic and it isn’t theoretical. Here’s what actually happens during a session, and why the effect keeps building after treatment ends.

Conditions We Treat With TMS
TMS isn’t a first-line treatment for everything. We recommend it when the evidence and your history point clearly to a good chance it will help.
This is the reason most patients come to us for TMS. If one or more antidepressants haven’t worked, or haven’t worked well enough, this is likely the conversation to have.
Depression careFor adults whose OCD keeps returning even with therapy and medication, deep TMS is FDA-cleared as an added treatment. It doesn’t replace ERP therapy, it works alongside it.
Request evaluationDepression tangled up with anxiety is uniquely exhausting and uniquely hard to treat. TMS is FDA-cleared for exactly that presentation.
Request evaluationGrowing evidence supports TMS as part of a broader psychiatric plan for PTSD, especially when depression is also present.
PTSD careConsidered case-by-case for adults with persistent anxiety that hasn’t responded to standard treatment. Requires an individual evaluation with Dr. Segal.
Request evaluationDeep TMS is FDA-cleared as an aid for adults trying to quit smoking short-term. Available on evaluation for the right candidates.
Request evaluationNot sure if TMS is right for you? Our Scottsdale team also offers Spravato and IV ketamine for treatment-resistant depression, alongside full psychiatric care. Call (623) 288-2628 and we’ll help you figure out the next step.
Why SMHWI Scottsdale
TMS works best when it’s one part of a coordinated plan. At our Scottsdale clinic, the same team that delivers your TMS also manages your medication and follows up on how you’re doing.
Every candidate is evaluated by Dr. Segal, a double board-certified psychiatrist, not a nurse working from a checklist.
Because we’re a full psychiatric practice, your TMS provider is the team managing your medications too.

We verify benefits, secure authorizations, and tell you your out-of-pocket cost before your first session.

Insurance & Cost
For most adults with depression that hasn’t responded to medication, insurance covers most of the treatment. Coverage typically requires proof that at least one antidepressant didn’t work. That’s paperwork our admissions team handles, you don’t have to chase it.
Frequently Asked Questions
Yes. Our Scottsdale clinic is at 8350 E Raintree Dr, Ste 125 & 130. We offer FDA-cleared TMS for depression that hasn’t responded to medication, for OCD, and for depression that comes with heavy anxiety. Dr. Roland Segal, MD oversees TMS at this location. Call (623) 288-2628 to see if it fits you.
For most adults with depression that hasn’t responded to medication, yes. Major commercial plans, Medicare, and Tricare cover TMS as long as you’ve tried at least one antidepressant that didn’t work. Prior authorization is usually required. Our team handles it before your first session and confirms your out-of-pocket cost.
Each session runs about 19 to 37 minutes, depending on the protocol prescribed for you. You’ll come in five days a week for about six weeks, followed by a brief taper. You’re awake the whole time, and you can drive yourself home right after.
Most patients describe it as a tapping or clicking sensation on the scalp. Mild scalp discomfort or a headache is common in the first week and usually fades within a few sessions. No pain, no sedation.
The most common thing patients notice is mild scalp discomfort or a headache during the first week, both of which usually settle down as treatment continues. Unlike antidepressants, TMS doesn’t cause weight gain, sexual side effects, fatigue, stomach problems, or emotional flatness.
Most patients start noticing changes in sleep, energy, and mental clarity within the first two to three weeks. Mood usually catches up in weeks three or four, and improvement often keeps building for weeks after treatment ends.
In most cases, yes. TMS works alongside virtually every psychiatric medication, and many patients stay on their current antidepressants throughout treatment. Dr. Segal reviews everything you’re taking at your evaluation.
ECT uses electrical current, requires anesthesia, and induces a controlled seizure. TMS uses focused magnetic pulses, no anesthesia, no seizure. You’re awake, you drive yourself home, and you’re back to your day.
TMS is FDA-cleared for adolescents ages 15 and older for major depression. Whether it’s appropriate for a specific teenager depends on their history, and coverage varies by plan.

Deborah was a nurse for 19 years before becoming a Board Certified Psychiatric Mental Health Nurse Practitioner. Deborah became a Licensed Practical Nurse in 2004 and then graduated as a Registered Nurse with her Associate of Applied Science Degree in Nursing in 2012 from Excelsior College. In 2019 Deborah earned her Bachelor of Science in Nursing from Capella University. Then Deborah graduated from Walden University in 2022 where she received her Master of Science in Nursing/Psychiatric Mental Health.
Before starting her journey of 7 years as an RN in mental health, Deborah worked in a variety of medical settings such as ER, Urgent Care, Oncology, Hospice, Med-Surg., Phone Triage, etc. But it was while working in the jail setting as a traveling nurse that she found her interest and passion for mental health since the prevalence of mental disorders within its population is high.
She then realized that many of the inmates were incarcerated for behaviors caused by their mental illness and that these types of facilities are not meeting the demand for treatment of psychiatric disorders. After finishing her travel assignment, Deborah decided to start working as a Psychiatric nurse and worked for institutions including Banner Behavioral Health, Phoenix Childrens Hospital, and Arizona State Hospital where she obtained experience with the children, adolescents, and adult population.
Deborah is passionate about mental health since she believes that individuals’ mental well-being is just as important as physical health. She has seen the impact it can have on individuals and society. Mental health is an often an overlooked aspect of overall health, and she believes that it is time for society to pay attention to it. Deborah believes that through advocacy she can work towards reducing the stigma surrounding mental health, ensuring that individuals receive the help they need without judgment or discrimination.
Deborah is eager to empower individuals to seek help, promote early intervention, and foster a supportive environment where her patients can thrive mentally and emotionally.
Hi! I am Arianna Saenz-Ochoa, a licensed professional counselor, and my journey towards helping others started when I earned my Bachelor’s degree in Elementary Education & Special Education from Arizona State University and Master’s degree in Clinical Mental Health Counseling from Grand Canyon University. I am a EMDRIA Certified EMDR Therapist and have completed advanced trainings and continue consultation in the application of EMDR with a focus on parts work, dissociation, and complex-attachment trauma.
My experience in providing therapy is mainly based in aiding others to find post-traumatic growth from childhood trauma and neglect, sexual abuse, and recovery from relationships with unhealthy dynamics of power / control. This also includes symptoms such as anxiety, depression, and self-harming behaviors. Most of my experience has been working in the outpatient individual setting with adults and teens.
I utilize a combination of skill developing approaches such as CBT, DBT, Parts Work, psychoeducation, and mindfulness training. As a result of client’s gaining enhanced self-awareness and lifestyle shifts, I encourage them when ready to process past negative experiences, utilizing the evidence-based therapy of EMDR, for sustained treatment progress.
I chose this profession due my own personal journey of post-traumatic growth from childhood trauma and negative cycles of behaviors. With lived experience, I find it an honor to create a meeting space where my clients can feel less judgement, be asked the tough questions, and release what pains them to find resolution.
Being born and raised in Arizona, you can find me escaping the heat with an iced coffee in hand, A/C blasting, and getting lost in a good book. You can also find me at community events supporting marginalized and oppressed folks, or on sniff-walking adventures with my pug mix, Lulu. These moments keep me balanced and energized to support my clients.
