What Is TMS Therapy

A different way to treat depression, and one that doesn’t rely on medication

Depression that hasn’t responded to medication is exhausting in a way that’s hard to explain to people who haven’t been there. You’ve tried the pills. Maybe two, maybe five. Some helped a little. Some didn’t help at all. Most came with side effects that made you wonder if the trade was worth it.

TMS is different. It’s a drug-free treatment that uses gentle magnetic pulses to reach the exact part of your brain that regulates mood, which is often quieter than it should be in people with depression. There’s nothing to swallow, nothing to metabolize, and none of the weight gain, sexual side effects, or emotional flatness that make so many antidepressants hard to live with.

TMS has been FDA-cleared for major depression since 2008, and later cleared for OCD and depression with anxious features. It’s most often prescribed for adults who’ve tried at least one antidepressant without getting the relief they hoped for. If that sounds like where you are, TMS might be worth talking about.

At our Rockwall clinic on Ralph Hall Parkway, sessions happen in a private, quiet room. You sit in a comfortable chair, closer to a dentist chair than a hospital bed, and stay awake the whole time. Read, listen to music, or just rest. When you’re done, you drive yourself home. Call (469) 707-9603 to see if TMS is right for you.

How TMS Works

The actual mechanism, in plain language

TMS isn’t magic and it isn’t theoretical. Here’s what actually happens during a session, and why the effect keeps building after treatment ends.

1Where it startsA small, precisely engineered coil rests against your scalp above the part of the brain that regulates mood. No needles. Nothing invasive.
2What happens insideThe coil sends short magnetic pulses through your skull, which you feel as light taps, and gently activates the brain circuits that have gone quiet.
3Why it lastsOver several weeks, those repeated activations strengthen the connections between the parts of your brain that manage mood. Your brain literally rewires itself.
4What you’ll noticeSleep, energy, and mental clarity usually shift first. Mood catches up in weeks three and four. Improvement often keeps building after treatment ends.
5The daily routineSessions run five days a week for about six weeks. Each one takes 19 to 37 minutes. You’re awake, comfortable, and drive yourself home right after.
6How we track progressStandardized rating scales at regular check-ins mean we’re not guessing. If something needs adjusting, we adjust it.

Conditions We Treat With TMS

What TMS is for, and what it isn’t

TMS isn’t a first-line treatment for everything. We recommend it when the evidence and your history point clearly to a good chance it will help.

Primary Indication

Depression that hasn’t lifted

This is the reason most patients come to us for TMS. If one or more antidepressants haven’t worked, or haven’t worked well enough, this is likely the conversation to have.

Why SMHWI Rockwall

Integrated psychiatric care, not just a TMS clinic

TMS works best when it’s one part of a coordinated plan. At our Rockwall clinic, the same team that delivers your TMS also manages your medication and follows up on how you’re doing.

A doctor decides fit

Every candidate is evaluated by a board-certified psychiatrist, not a nurse working from a checklist.

One team, one plan

Because we’re a full psychiatric practice, your TMS provider is the team managing your medications too.

Close to home

Right off I-30 at Ralph Hall Parkway. No Dallas commute on top of an already-hard day.

Frequently Asked Questions

TMS therapy at SMHWI Rockwall

Yes. Our Rockwall clinic is at 810 E Ralph Hall Pkwy, Ste 140. We offer FDA-cleared TMS for depression that hasn’t responded to medication, for OCD, and for depression that comes with heavy anxiety. Dr. Ryan H. Hashem, MD oversees TMS at this location. Call (469) 707-9603 to see if it fits you.

For most adults with depression that hasn’t responded to medication, yes. Major commercial plans, Medicare, Tricare, and many Texas Medicaid plans cover TMS, as long as you’ve tried at least one antidepressant that didn’t work. Prior authorization is usually required. Our team checks your specific plan, gets the authorizations needed, and tells you your out-of-pocket cost before your first session.

Each session runs about 19 to 37 minutes, depending on the protocol prescribed for you. You’ll come in five days a week for about six weeks, followed by a brief taper. You’re awake the whole time, and you can drive yourself home right after.

Most patients describe it as a tapping or clicking sensation on the scalp. Mild scalp discomfort or a headache is common in the first week and usually fades within a few sessions. No pain, no sedation.

The most common thing patients notice is mild scalp discomfort or a headache during the first week, both of which usually settle down as treatment continues. Unlike antidepressants, TMS doesn’t cause weight gain, sexual side effects, fatigue, stomach problems, or emotional flatness.

Most patients start noticing changes in sleep, energy, and mental clarity within the first two to three weeks. Mood usually catches up in weeks three or four, and improvement often keeps building for weeks after treatment ends.

In most cases, yes. TMS works alongside virtually every psychiatric medication, and many patients stay on their current antidepressants throughout treatment. Dr. Hashem reviews everything you’re taking at your evaluation.

ECT uses electrical current, requires anesthesia, and induces a controlled seizure. TMS uses focused magnetic pulses, no anesthesia, no seizure. You’re awake, you drive yourself home, and you’re back to your day.

TMS is FDA-cleared for adolescents ages 15 and older for major depression. Whether it’s appropriate for a specific teenager depends on their history, and coverage varies by plan.

Deborah Baum

Deborah Baum, PMHNP,

Deborah was a nurse for 19 years before becoming a Board Certified Psychiatric Mental Health Nurse Practitioner. Deborah became a Licensed Practical Nurse in 2004 and then graduated as a Registered Nurse with her Associate of Applied Science Degree in Nursing in 2012 from Excelsior College. In 2019 Deborah earned her Bachelor of Science in Nursing from Capella University. Then Deborah graduated from Walden University in 2022 where she received her Master of Science in Nursing/Psychiatric Mental Health.

Before starting her journey of 7 years as an RN in mental health, Deborah worked in a variety of medical settings such as ER, Urgent Care, Oncology, Hospice, Med-Surg., Phone Triage, etc. But it was while working in the jail setting as a traveling nurse that she found her interest and passion for mental health since the prevalence of mental disorders within its population is high.

She then realized that many of the inmates were incarcerated for behaviors caused by their mental illness and that these types of facilities are not meeting the demand for treatment of psychiatric disorders. After finishing her travel assignment, Deborah decided to start working as a Psychiatric nurse and worked for institutions including Banner Behavioral Health, Phoenix Childrens Hospital, and Arizona State Hospital where she obtained experience with the children, adolescents, and adult population.

Deborah is passionate about mental health since she believes that individuals’ mental well-being is just as important as physical health. She has seen the impact it can have on individuals and society. Mental health is an often an overlooked aspect of overall health, and she believes that it is time for society to pay attention to it. Deborah believes that through advocacy she can work towards reducing the stigma surrounding mental health, ensuring that individuals receive the help they need without judgment or discrimination.

Deborah is eager to empower individuals to seek help, promote early intervention, and foster a supportive environment where her patients can thrive mentally and emotionally.
