We appreciate the importance of your privacy, and protect your personal information from any inappropriate, unauthorized uses or disclosures. This notice describes how your medical information may be used and disclosed, and how you can get access to this information. Uses and disclosures not described in the notice will be made only with your written consent. Please review it carefully. 

Your Rights

Your Rights

When it comes to your health information, you have certain rights. This section explains your rights and some of our responsibilities to help you.

Get an electronic or paper copy of your medical record

You can ask to see or get an electronic or paper copy of your medical record and other health information we have about you. We will provide a copy or a summary of your health information, usually within 30 days of your request. We may charge a reasonable, cost-based fee.

Ask us to correct your medical record

You can ask us to correct health information about you that you think is incorrect or incomplete. We may say “no” to your request, but we’ll tell you why in writing within 60 days.

Request confidential communications

You can ask us to contact you in a specific way or place (for example, home or office, mail or telephone). We will say “yes” to all reasonable requests.

Ask us to limit what we use or share

  • You can ask us not to use or share certain health information for treatment, payment, or our operations. You may provide a single consent for all future uses or disclosures for treatment, payment, and health care operations purposes. We are not required to agree to your request, and we may say “no” if it would affect your care.
  • If you pay for a service or health care item out-of-pocket in full, you can ask us not to share that information for the purpose of payment or our operations with your health insurer. We will say “yes” unless a law requires us to share that information.
  • You may revoke your consent at any time.

 

Get a list of those with whom we’ve shared information

You can ask for a list (accounting) of the times we’ve shared your health information for six years prior to the date you ask, who we shared it with, and why. We will include all the disclosures except for those about treatment, payment, and health care operations, and certain other disclosures (such as any you asked us to make). We’ll provide one accounting a year for free but will charge a reasonable, cost-based fee if you ask for another one within 12 months.

Get a copy of this privacy notice

You can ask for a paper copy of this notice at any time, even if you have agreed to receive the notice electronically. We will provide you with a paper copy promptly.

Choose someone to act for you

If you have given someone medical power of attorney or if someone is your legal guardian, that person can exercise your rights and make choices about your health information. We will make sure the person has this authority and can act for you before we take any action.

File a complaint if you feel your rights are violated

  • You can complain if you feel we have violated your rights. Please contact our Privacy Officer at COMPLIANCE@TURNWELLMENTALHEALTH.COM.
  • You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights by sending a letter to 200 Independence Avenue, S.W., Washington, D.C. 20201, calling 1-877-696-6775, or visiting hhs.gov/ocr/privacy/hipaa/complaints/.
  • We take all complaints seriously, and will look into it immediately. We will not retaliate against you for filing a complaint.

 

Your Choices

For certain health information, you can tell us your choices about what we share. If you have a clear preference for how we share your information in the situations described below, talk to us. Tell us what you want us to do, and we will follow your instructions.

In these cases, you have both the right and choice to tell us to:

  • Share information with your family, close friends, or others involved in your care
  • Share information in a disaster relief situation

If you are not able to tell us your preference, for example if you are unconscious, we may go ahead and share your information if we believe it is in your best interest. We may also share your information when needed to lessen a serious and imminent threat to health or safety.

In these cases we never share your information unless you give us written permission:

  • Marketing purposes
  • Sale of your information
  • Most sharing of mental health-related notes

 

How do we Typically Use or Share your Health Information?

We typically use or share your health information in the following ways:

Treat you

We can use your health information and share it with other professionals who are treating you. Example: A doctor treating you for an injury asks another doctor about your overall health condition.

Run our organization

We can use and share your health information to run our practice, improve your care, and contact you when necessary. Example: We use health information about you to manage your treatment and services.

Bill for your services

We can use and share your health information to bill and get payment from health plans or other entities. Example: We give information about you to your health insurance plan so it will pay for your services.

How else can we use or share your health information?

We are allowed or required to share your information in other ways – usually in ways that contribute to the public good, such as public health and research. We have to meet many conditions in the law before we can share your information for these purposes.

Help with public health and safety issues

We can share health information about you for certain situations such as:

  • Preventing disease
  • Helping with product recalls
  • Reporting adverse reactions to medications
  • Reporting suspected abuse, neglect, or domestic violence
  • Preventing or reducing a serious threat to anyone’s health or safety

 

Do research 

We can use or share your information for health research.

Comply with the law

We will share information about you if state or federal laws require it, including with the Department of Health and Human Services if it wants to see that we’re complying with federal privacy law.

Respond to organ and tissue donation requests

We can share health information about you with organ procurement organizations.

Work with a medical examiner or funeral director

We can share health information with a coroner, medical examiner, or funeral director when an individual dies.

Address workers’ compensation, law enforcement, and other government requests

We can use or share health information about you:

  • For workers’ compensation claims
  • For law enforcement purposes or with a law enforcement official
  • With health oversight agencies for activities authorized by law
  • For special government functions such as military, national security, and presidential protective services

 

Respond to lawsuits and legal actions

We can share health information about you in response to a court or administrative order, or in response to a subpoena.

Our Responsibilities

  • We are required by law to maintain the privacy and security of your protected health information.
  • We will let you know promptly if a breach occurs that may have compromised the privacy or security of your information.
  • We must follow the duties and privacy practices described in this notice and give you a copy of it.
  • We will not use or share your information other than as described here unless you tell us we can in writing. If you tell us we can, you may change your mind at any time. Let us know in writing if you change your mind.

 

Changes to the Terms of this Notice

We can change the terms of this notice, and the changes will apply to all information we have about you. The new notice will be available upon request, in our office, and on our web site.

Expiration Date

You consent will expire when you revoke it.

Deborah Baum

Deborah Baum, PMHNP,

Deborah was a nurse for 19 years before becoming a Board Certified Psychiatric Mental Health Nurse Practitioner. Deborah became a Licensed Practical Nurse in 2004 and then graduated as a Registered Nurse with her Associate of Applied Science Degree in Nursing in 2012 from Excelsior College. In 2019 Deborah earned her Bachelor of Science in Nursing from Capella University. Then Deborah graduated from Walden University in 2022 where she received her Master of Science in Nursing/Psychiatric Mental Health.

Before starting her journey of 7 years as an RN in mental health, Deborah worked in a variety of medical settings such as ER, Urgent Care, Oncology, Hospice, Med-Surg., Phone Triage, etc. But it was while working in the jail setting as a traveling nurse that she found her interest and passion for mental health since the prevalence of mental disorders within its population is high.

She then realized that many of the inmates were incarcerated for behaviors caused by their mental illness and that these types of facilities are not meeting the demand for treatment of psychiatric disorders. After finishing her travel assignment, Deborah decided to start working as a Psychiatric nurse and worked for institutions including Banner Behavioral Health, Phoenix Childrens Hospital, and Arizona State Hospital where she obtained experience with the children, adolescents, and adult population.

Deborah is passionate about mental health since she believes that individuals’ mental well-being is just as important as physical health. She has seen the impact it can have on individuals and society. Mental health is an often an overlooked aspect of overall health, and she believes that it is time for society to pay attention to it. Deborah believes that through advocacy she can work towards reducing the stigma surrounding mental health, ensuring that individuals receive the help they need without judgment or discrimination.

Deborah is eager to empower individuals to seek help, promote early intervention, and foster a supportive environment where her patients can thrive mentally and emotionally.

Lamia Abdouni, PMHNP

My name is Lamia Abdouni, and I am a board certified Psychiatric Mental Health Nurse Practitioner with a background in both family and psychiatric nursing. I became a Registered Nurse in 2007 after earning my nursing degree from Mercy College of Northwest Ohio. In 2016, I obtained my Family Nurse Practitioner degree from Walden University, and in 2019, I earned my Psychiatric Mental Health Nurse Practitioner degree from Binghamton University. I have been working in the field of mental health since 2014.

I specialize in the evaluation and treatment of a wide range of psychiatric disorders, with a particular focus on depression, anxiety disorders, and trauma-related conditions. I consider myself an interventional healthcare provider and believe in utilizing comprehensive treatment approaches tailored to each individual’s needs. My areas of focus include higher levels of care such as Partial Hospitalization Programs (PHP), Intensive Outpatient Programs (IOP), and residential treatment. I also have a strong interest and focus in innovative psychiatric treatments, including Transcranial Magnetic Stimulation (TMS) and ketamine therapy.

I chose this profession because I have always been passionate about helping people navigate some of the most challenging periods of their lives. I believe that mental health care requires compassion, understanding, and a commitment to finding effective, individualized solutions. The most rewarding part of my work is witnessing patients regain hope, improve their quality of life, and achieve meaningful progress in their mental health journey.

Outside of work, I enjoy swimming, roller skating, and strength training. These activities help me maintain balance, resilience, and energy, allowing me to continue providing thoughtful and compassionate care to my clients.

Danielle Siragusa, LCSW, is a behavioral health leader with a strong track record of building and scaling high-impact clinical programs. As Vice President of Behavioral Health, she brings a unique blend of clinical expertise, operational strategy, and people-centered leadership to her work, with a focus on delivering accessible, high-quality care.

Danielle began her career as a clinician, which continues to shape her leadership approach today. As a Licensed Clinical Social Worker, she has extensive experience working with individuals and families across a range of behavioral health needs. Her clinical foundation allows her to design programs that are not only evidence-based, but also practical, compassionate, and responsive to the real-world needs of clients and providers.

Throughout her career, Danielle has held progressive leadership roles, where she has led multidisciplinary teams, developed integrated care models, and overseen complex behavioral health operations. She is known for her ability to translate vision into execution, whether that’s launching new service lines, optimizing clinical workflows, or driving system-wide improvements in quality, access, and outcomes. Jeremy Edwards

My name is Jeremy Edwards, I am board-certified Psychiatric Mental Health Nurse Practitioner with over 15 years of experience in the behavioral health field, working across a wide range of settings including schools, group homes, residential treatment centers, outpatient clinics, and inpatient hospitals. I earned my Bachelor’s degree in Psychology from the University of Colorado, followed by a Bachelor of Science in Nursing and a Doctorate of Nursing Practice degree from Creighton University.
 
My passion for mental health care is deeply rooted in my own life experiences. Having overcome significant adversity, I was inspired to become part of the solution and to support others on their healing journeys. Jose earned his Bachelor of Science in Nursing from Oregon Health & Science University in 2013, followed by a Master of Science in Nursing with a specialization in Psychiatric Mental Health in 2021 from Frontier Nursing University.

Specializing in treating patients across their lifespan, Jose is particularly passionate about supporting military veterans dealing with PTSD, depression, anxiety, and substance abuse. With 9 years of experience as a critical care nurse and crisis nurse in Boise, Idaho, and a total of 20 years in the United States Army Infantry, including six years in active duty, Jose brings a unique and comprehensive perspective to his practice.

Jose is an Army Infantry Combat Veteran and retired Honorably as a Sergeant First Class from the United States Army after 20 years of faithful service.  He spent time in the Middle East, Korea, and numerous places in the United States.

Not only does Jose provide evidence based pharmacological therapies to help patients struggling with their mental health, but he also utilizes a patient centered approach which individualizes the plan of care to the patient.

I specialize in anxiety, depression, and substance use disorders, working primarily with adults with co-occurring disorders. My approach to care is collaborative and strengths-focused, emphasizing each client’s unique resilience and capacity for growth. I often use cognitive-behavioral therapy (CBT), and mindfulness-based interventions to help clients build insight and develop practical coping skills.

I chose this profession because I have a family member who struggled with mental health challenges, and that experience showed me how important compassion and understanding are in the healing process. Angela Ross, MS, LPC, is a Licensed Professional Counselor and a proud Arizona native. She earned her Master’s degree in Professional Counseling from Grand Canyon University in 2020. Angela has extensive experience working with individuals facing mental health challenges in both outpatient and inpatient settings. She specializes in working with adolescents, adults, and families, utilizing a variety of therapeutic approaches, including CBT, DBT, EMDR, motivational interviewing, and solution-focused therapy.

With a robust background in both individual and group counseling, Angela creates a supportive, collaborative space where clients feel safe to engage in meaningful self-exploration and growth. I chose this profession because of my deep belief in compassion, resilience and the power of human connection, and I find it most rewarding to witness my client’s growth and healing, seeing them gain insight, strength and hope as they create positive changes in their lives.

Dr. Roland Segal, M.D., Managing Partner, Scottsdale Psychiatrist

Dr. Roland Segal, a distinguished psychiatrist, holds the role of Managing Partner and MD Psychiatrist at Synergy Mental Health and Wellness Integrated. Graduating from the University of Arizona, College of Medicine, he pursued his general psychiatry training at Banner Good Samaritan Medical Center in Phoenix, Arizona. Dr. Segal further refined his skills through a forensic psychiatry fellowship at the University of Southern California, Keck School of Medicine.

Dr. Segal has more than 10 years of clinical, administrative, executive and forensic experience. With double board certification in General and Forensic Psychiatry, Dr. Segal brings a wealth of knowledge and experience to our team. He served as the Chief Medical Officer at Valley Hospital in Phoenix, Arizona, president of the Arizona Psychiatric Society, chair of the legislative committee and a member of multiple state and national boards, committees and organizations. Dr. Segal is a clinical assistant professor of psychiatry at the University of Arizona, College of Medicine, where he regularly teaches medical students and residents.

Dr. Segal serves as a psychiatry expert consultant for a number of superior and regional courts including Salt River, Maricopa and Yuma County; city governments including Phoenix, Lake Havasu and Mesa; and organizations including the United Postal Service, Social Security Administration, Immigration Health Services, US Department of Justice and the Department of Homeland Security.
