The Admissions Hub

One admissions team, every SMHWI IOP program for Arizona and Texas

Finding the right IOP shouldn’t be a project. The SMHWI admissions team at Synergy Mental Health & Wellness Integrated (SMHWI) exists to take the procedural work off your plate: insurance verification, benefits explanation, confidential clinical assessment, treatment-plan scheduling, and placement into the IOP program that actually fits what you’re dealing with.

This page is the enrollment front door for all of our intensive outpatient programs: adult mental health, eating disorders, women’s IOP, college-student IOP, virtual IOP in both Arizona and Texas, and in-person IOP at our Tempe, Glendale, Scottsdale, and Rockwall clinics. For the clinical details of any specific program (curriculum, schedule, conditions treated), head to that program’s page; the links are below. For how to actually start, keep reading.

Under the federal Mental Health Parity and Addiction Equity Act, most insurance plans must cover IOP on equal terms with medical care. Our admissions team confirms your specific plan at no cost before you commit to anything. No referral required. Most patients begin within days.

Which Program Fits You

Where to go after admissions: our IOP programs across AZ & TX

Admissions is the front door, not the program itself. During intake, our clinical team confirms which SMHWI IOP is the right match for your diagnosis, your schedule, and the level of care you need. Each program has its own page with the curriculum, calendar, conditions treated, and location details. That’s where to look for program-specific answers.

If you already know which program you want, follow the link to that page and start intake there (same form, same team). If you’re not sure, start with this admissions page and we’ll route you.

If IOP isn’t the right level of care, we’ll say so during the assessment and point you toward standard outpatient therapy, psychiatric evaluation, PHP referral, or the right next step.

The 5-Step Enrollment

From first call to first session, step by step

Our enrollment process is designed to be fast, transparent, and low-barrier. Most patients move from initial contact to first IOP session within days, with no referral required and no out-of-pocket surprise at the end.

Step & Timing
What happens
What you need to do
1 · Same day
You call or submit the online form. An admissions coordinator responds the same business day, gathers basic info, and answers your questions about IOP, insurance, and next steps.
Call the clinic nearest you or submit the intake form above. Have your insurance card handy if possible (not required to start the conversation).
2 · Within 24 hrs
We verify your IOP benefits with your insurance carrier at no cost: in-network status, deductible, co-insurance, authorization requirements, and session limits.
Nothing. We handle the carrier calls and come back with a plain-language benefits breakdown and your exact out-of-pocket responsibility, if any.
3 · 1 to 2 days
A licensed clinician conducts a confidential 60 to 90 minute clinical assessment to confirm IOP is the right level of care and identify the right program track.
Attend the intake in person at your closest clinic or over secure video. Bring a photo ID, insurance card, and a list of current medications.
4 · Same day as intake
The clinical team builds your individualized treatment plan and confirms your program, start date, and session schedule. You’re matched to the SMHWI IOP that fits your needs and your daily life.
Confirm your start date and preferred session track. If you’re starting virtually, we also walk you through the telehealth platform here.
5 · Within days
You attend your first IOP session in person at Tempe, Glendale, Scottsdale, or Rockwall, or virtually from anywhere in Arizona or Texas. Your clinical team already has your assessment on file.
Show up (or sign in). Everything you need (group assignment, clinician, treatment plan) is in place from day one.

Insurance & Cost

Does insurance cover IOP? What will it cost?

The short answer: most major plans cover IOP when medically necessary, and federal law requires parity with medical coverage. The specific answer depends on your plan, your deductible, and your authorization. We verify both before you commit to anything.

Federal Mental Health Parity

The Mental Health Parity and Addiction Equity Act (MHPAEA) requires most group and individual plans to cover mental health and substance use treatment, including IOP, on terms no more restrictive than medical and surgical coverage. Your insurer cannot impose stricter deductibles, co-pays, or session limits on IOP than on comparable medical care.

In-Network Carriers

SMHWI is in-network with most major commercial, Medicare, and TRICARE plans: Aetna, Blue Cross Blue Shield (including BCBS of Arizona and BCBS of Texas), Cigna, Evernorth, Humana, Magellan Health, UnitedHealthcare, Medicare, and TRICARE. If your carrier isn’t listed, ask: the network changes.

Free Benefits Verification

We call your carrier for you, confirm IOP coverage, and document your deductible, co-insurance percentage, authorization requirements, and any session limits. You get a plain-language breakdown and your exact patient responsibility before your first session. No surprise bills.

Authorization & Program Length

Most carriers require prior authorization for IOP and reauthorize based on clinical progress. Program length is set by your plan and your clinical trajectory, not by us. Our utilization team manages the authorization and reauthorization work throughout your treatment so sessions aren’t interrupted.

Your Out-of-Pocket

After insurance, your cost depends on your deductible, your co-insurance rate, and whether you’ve already met your plan’s out-of-pocket maximum this year. Patients who have already hit their max for medical care in the same plan year often pay very little or nothing for IOP. We quote the exact number before you start.

Self-Pay & Payment Plans

If you’re uninsured, your plan doesn’t cover IOP, or benefits are limited, we discuss self-pay rates and payment plan options openly. We work with every patient on a financially accessible path to care. Cost should not be the thing that keeps you out of treatment.

What to Expect at Intake

Your clinical assessment, demystified

The intake assessment is a confidential 60 to 90 minute conversation with a licensed clinician. It’s not a test, there’s no right answer, and nothing you share goes anywhere outside your treatment team. Here’s exactly what it covers.

Why you’re here now

What’s been happening, when it started, and what prompted you to reach out. There’s no judgment and no need to have the right words. The clinician does the organizing.

Mental health & treatment history

Prior diagnoses, past therapy or psychiatric care, hospitalizations, medications that have helped or not. If you don’t remember exactly, approximate dates and provider names are fine.

Medical & medication context

Current medications and dosages (prescription and over-the-counter), relevant medical conditions, allergies. If you’re on a med list, bring it or screenshot it.

Daily-life context

Work, school, family, living situation, and what’s been affected. This helps the clinician recommend the right program track (morning vs. afternoon, in-person vs. virtual) and the right level of care.

Your treatment goals

What you want to be different in three months. These shape your individualized plan and give your clinical team a shared target. “I don’t know” is also a valid starting point. The clinician helps you find it.

Level-of-care recommendation

At the end, the clinician confirms whether IOP is the right fit, identifies which SMHWI program and track to place you in, and gives you a clear next step. If IOP isn’t appropriate, you’ll leave with a referral.

In-person or virtual? Both start here

Whether you’re enrolling at one of our four clinics or in Virtual IOP (statewide in Arizona or Texas), the admissions path is the same: free insurance verification, confidential intake, and placement into the right program. Our team routes you to the correct track during the assessment, so you don’t have to decide in advance.

What to Bring & What’s Next

Getting ready for your intake and first week

Enrollment is low-friction on purpose. Here’s the short list of what to have ready for intake, and what your first week in IOP looks like once you’re enrolled.

What to bring to your intake

You don’t need everything on this list to start, but having these on hand makes intake smoother. If you’re missing items, we proceed and gather them as we go.

1

Photo ID & insurance card

Required

Driver’s license, passport, or state ID. Front and back of your insurance card (physical or screenshot on your phone). If you’re on a parent or spouse’s plan, the policyholder’s info.

2

Current medication list

Recommended

Prescription medications, dosages, prescribing provider, and anything over-the-counter you take regularly. Pharmacy printout or app screenshot works.

3

Prior clinical records

Helpful if available

Recent discharge summary, prior IOP/PHP records, assessments, or a referral note from a current provider. If you don’t have them, we can request records on your behalf with a signed release.

4

A couple of questions

Optional

Anything you want to ask about the program, scheduling, confidentiality, or treatment approach. Intake is a two-way conversation, and it’s how we both decide this is the right fit.

Your first week in IOP

The first week focuses on getting comfortable and building your foundation. You won’t be expected to share more than you’re ready to.

Orientation & introduction

Meet your clinical team and fellow group members. Review program expectations, scheduling, and confidentiality guidelines.

Treatment planning

Work with your therapist to set personalized treatment goals based on your assessment and clinical needs.

Group & individual sessions

Begin attending group therapy and individual counseling as part of your structured weekly schedule.

Psychiatric consultation

If applicable, meet with a psychiatric provider for medication evaluation and management as part of your overall plan.

Frequently Asked Questions

IOP admissions & insurance, answered

Most major insurance plans cover IOP when medically necessary. The federal Mental Health Parity and Addiction Equity Act (MHPAEA) requires insurance plans to cover mental health and substance use treatment on equal terms with medical and surgical coverage, so insurers cannot impose stricter limits on IOP than they apply to comparable medical treatments. Our admissions team verifies your specific benefits at no cost before enrollment.

SMHWI is in-network with most major commercial, Medicare, and TRICARE plans, including Aetna, Blue Cross Blue Shield (including BCBS of Arizona and BCBS of Texas), Cigna, Evernorth, Humana, Magellan Health, UnitedHealthcare, Medicare, and TRICARE. We verify your specific plan at no charge during the admissions process.

Your IOP cost depends on your insurance plan, your deductible, your co-insurance percentage, and whether you have met your out-of-pocket maximum for the year. For most insured patients, IOP is covered when medically necessary under federal mental health parity laws. Our admissions team provides a plain-language cost breakdown based on your specific plan before your first session. If you’re uninsured or your plan doesn’t cover IOP, we discuss self-pay rates and payment plans.

Most patients begin IOP within days of first contact. Our admissions team completes insurance verification within 24 business hours, schedules the clinical assessment within 1 to 2 days, and places you in a program the same day as the assessment. Call (623) 288-3656 (AZ) or (469) 707-9603 (TX) to get started.

No referral is required. You can self-refer by calling any SMHWI location or submitting the online intake form. We also accept referrals from primary care providers, therapists, psychiatrists, hospitals, schools, and family members.

The intake assessment is a confidential 60 to 90 minute clinical evaluation conducted by a licensed clinician. It covers your presenting concerns, mental health and treatment history, current medications, treatment goals, and any relevant medical considerations. The assessment confirms whether IOP is the right level of care and identifies the specific SMHWI program track that fits you. It can be completed in person or over secure video.

Partial Hospitalization Programs (PHP) provide more intensive daily treatment: typically 5 days per week for 6 or more hours per day. Intensive Outpatient Programs (IOP) are less intensive: typically 3 days per week for 3 hours per session. IOP is appropriate for people who are medically stable and can manage daily responsibilities between sessions. Our admissions team determines the right level of care during intake. SMHWI offers IOP; patients needing PHP are referred to appropriate partners.

Our clinical team builds a step-down plan as you approach the end of IOP. It may include standard weekly outpatient therapy, ongoing medication management, support groups, and community resources. The goal is a supported, gradual transition that protects the progress you made in IOP.

Both. Arizona patients can enroll in in-person IOP at our Tempe, Glendale, or Scottsdale clinics, or in Virtual IOP Arizona statewide. Texas patients can enroll in in-person IOP at our Rockwall clinic or in Virtual IOP Texas statewide. Call (623) 288-3656 for Arizona admissions or (469) 707-9603 for Texas admissions.

A valid photo ID, your insurance card, a list of current medications and dosages, and any relevant clinical records or referrals from previous providers. If you don’t have all of these items, we can still proceed with the intake and gather what we need along the way.

Deborah Baum

Deborah Baum, PMHNP,

Deborah was a nurse for 19 years before becoming a Board Certified Psychiatric Mental Health Nurse Practitioner. Deborah became a Licensed Practical Nurse in 2004 and then graduated as a Registered Nurse with her Associate of Applied Science Degree in Nursing in 2012 from Excelsior College. In 2019 Deborah earned her Bachelor of Science in Nursing from Capella University. Then Deborah graduated from Walden University in 2022 where she received her Master of Science in Nursing/Psychiatric Mental Health.

Before starting her journey of 7 years as an RN in mental health, Deborah worked in a variety of medical settings such as ER, Urgent Care, Oncology, Hospice, Med-Surg., Phone Triage, etc. But it was while working in the jail setting as a traveling nurse that she found her interest and passion for mental health since the prevalence of mental disorders within its population is high.

She then realized that many of the inmates were incarcerated for behaviors caused by their mental illness and that these types of facilities are not meeting the demand for treatment of psychiatric disorders. After finishing her travel assignment, Deborah decided to start working as a Psychiatric nurse and worked for institutions including Banner Behavioral Health, Phoenix Childrens Hospital, and Arizona State Hospital where she obtained experience with the children, adolescents, and adult population.

Deborah is passionate about mental health since she believes that individuals’ mental well-being is just as important as physical health. She has seen the impact it can have on individuals and society. Mental health is an often an overlooked aspect of overall health, and she believes that it is time for society to pay attention to it. Deborah believes that through advocacy she can work towards reducing the stigma surrounding mental health, ensuring that individuals receive the help they need without judgment or discrimination.

Deborah is eager to empower individuals to seek help, promote early intervention, and foster a supportive environment where her patients can thrive mentally and emotionally.

Rosanne Charles,
DNP, PMHNP-BC

My name is Dr. Rosanne Charles, DNP, PMHNP-BC, and I am a Psychiatric Mental Health Nurse Practitioner at Synergy Mental Health & Wellness Integrated. I graduated from Florida International University and have dedicated more than two decades to a career in nursing and mental health care.

I specialize in psychiatric medication management for anxiety disorders, depression and other mood disorders, bipolar disorder, ADHD, and substance use concerns. I work with teens ages 14–17, adults, and older adults, and I have experience caring for diverse populations, including LGBTQ+ individuals, military members and veterans, and first responders.

My approach to care is compassionate, thoughtful, collaborative, and evidence-based. I believe effective mental health treatment should be both clinically sound and practical for everyday life. I take time to listen carefully, understand each patient’s unique circumstances, establish accurate diagnoses, and develop individualized treatment plans that support stability, clarity, and improved quality of life. In addition to medication management, I incorporate Cognitive Behavioral Therapy (CBT), mindfulness-based strategies, and principles of Acceptance and Commitment Therapy (ACT) to support long-term mental wellness.

I chose this profession because I have always wanted to make a difference in the lives of others. After beginning my nursing career more than two decades ago, I have found it deeply rewarding to support people through some of their most challenging moments and help them move forward with greater confidence and stability. What I find most rewarding about my work is helping others and knowing that the care I provide can make a meaningful difference in their lives.

Outside of work, I enjoy spending time with family, cooking, traveling, sightseeing, painting, dancing, and listening to music. I also enjoy staying active through biking, bowling, and mini golf. These interests allow me to maintain balance, creativity, and connection in my own life.

Arianna Saenz-Ochoa, MS, LPC

Hi! I am Arianna Saenz-Ochoa, a licensed professional counselor, and my journey towards helping others started when I earned my Bachelor’s degree in Elementary Education & Special Education from Arizona State University and Master’s degree in Clinical Mental Health Counseling from Grand Canyon University. I am a EMDRIA Certified EMDR Therapist and have completed advanced trainings and continue consultation in the application of EMDR with a focus on parts work, dissociation, and complex-attachment trauma.

My experience in providing therapy is mainly based in aiding others to find post-traumatic growth from childhood trauma and neglect, sexual abuse, and recovery from relationships with unhealthy dynamics of power / control. This also includes symptoms such as anxiety, depression, and self-harming behaviors. Most of my experience has been working in the outpatient individual setting with adults and teens.

I utilize a combination of skill developing approaches such as CBT, DBT, Parts Work, psychoeducation, and mindfulness training. As a result of client’s gaining enhanced self-awareness and lifestyle shifts, I encourage them when ready to process past negative experiences, utilizing the evidence-based therapy of EMDR, for sustained treatment progress.

I chose this profession due my own personal journey of post-traumatic growth from childhood trauma and negative cycles of behaviors. With lived experience, I find it an honor to create a meeting space where my clients can feel less judgement, be asked the tough questions, and release what pains them to find resolution.

Being born and raised in Arizona, you can find me escaping the heat with an iced coffee in hand, A/C blasting, and getting lost in a good book. You can also find me at community events supporting marginalized and oppressed folks, or on sniff-walking adventures with my pug mix, Lulu. These moments keep me balanced and energized to support my clients.
